We want your first visit to be smooth and comfortable. Here's everything you need to know before you arrive.

Call to Schedule

Reach us at 859-226-2500 during business hours. Our front desk team will find a time that works for you and walk you through what to bring.

Complete New Patient Forms

Arrive 15 minutes early to fill out our new patient paperwork, or complete it online ahead of time if available. Bring your photo ID and insurance card.

Your Initial Consultation

Your first visit is an in-depth conversation. We'll review your medical history, current concerns, and goals to build a care plan that's uniquely yours.

Ongoing Partnership

From there, we'll schedule follow-ups, coordinate referrals if needed, and be available between visits for questions and concerns. You're never just a chart number.
